New Applicants:
Thank you for you interest in Urban City Impact. Our mentorship program is currently at full capacity. To maintain the quality of support we provide, new applicants will be placed on a waitlist until space becomes available. We encourage you to submit your application. Someone from our team will contact you within two business days.
Attention Parents:
We must have a completed 2025 permission form on file in order for your child to participate in any mentorship outings.
If your child has moved, has updated health or health insurance information, or if you need to update your phone number or emergency contact details, please complete and submit an Information Update Form.
